The Preventing relapse module can be used by therapists with clients who have a range of mental health issues and may benefit from a relapse prevention plan. In mental healthcare, various terms are used for a relapse prevention plan, including a signalling plan.
Content of the module
Those who follow the module will map out their thoughts, sources of stress, signals, etc. Then, the phases (green, orange and red) are introduced. The goal of the module is to enable clients and their (social) environment to act appropriately in each phase. This increases their self-control and autonomy. A well-structured plan can also help shape how complaints are experienced.
The relapse prevention plan is a tool for preventing relapse. The answers clients provide in the module are collected in a separate plan, which can easily be accessed from the homepage and the app. This way, they always have it on hand when they need to view or update the plan.
What does the module consist of?
The module consists of several chapters with a number of feedback moments, where answers are sent to the client’s therapist who can then provide feedback. As always, therapists have the option to turn feedback moments on or off.

The module activates the 'Preventing relapse' diary and the 'Relapse prevention plan'. The answers clients provide about the green, orange, and red phases are automatically added to the relapse prevention plan. What is the module based on?
The Preventing relapse module was developed by Minddistrict in collaboration with involved partners. The content of the module was created through an extensive development process and is based on both relevant literature and the knowledge and experience of therapists and clients.
